This survey is being carried out by the Royal Alexandra Children's Hospital in Brighton in conjunction with IBDrelief.
A very limited amount is known about the diets of IBD patients 18 and under. This survey aims to gain more of an understanding about the diets of children living with IBD.
Any Crohn's disease or ulcerative colitis patient aged 18 and under in the UK is invited to take the survey. If you are under 16 we ask that you fill it in with the support of a parent or guardian.
If you are outside of the UK then we are sorry but you are not eligible to take part on this occasion.
Results will be published anonymously.
